29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) Main Roof, 1128 East State Street, Archbald, PA 18403 - On or about November 16, 2023, employees were exposed to a fall hazard of approximately eighteen (18) to thirty-two (32) feet from not utilizing a rope attached in the center of the wearer's back near the shoulder level. Abatement Certification and Documentation Required.
