29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: Milcon Construction Corp. did not record the following workplace injuries or illnesses on the OSHA 300 form or equivalent for the calendar year of 2023; a) Worksite, Southwest corner: On or about April 26, 2024, the employer failed to record a work-related injury that occurred on November 8, 2023. An employee fell approximately 18 feet to the ground while conducting roofing activities on a commercial building. The employee sustained a left femur fracture, left tibia avulsion fracture, and left side rib fracture.
