29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Northbrook Behavioral Health Hospital, 425 Woodbury-Turnersville Road, Blackwood, NJ 08012: On or about 1/23/2023, the employer did not record the following work-related injuries on the OSHA 300 Log for calendar year 2023: a) On or about 10/27/2023, a Registered Nurse Manager was scratched in the face by a patient which resulted in medical treatment beyond first aid. b) On or about 11/1/2023, a Mental Health Associate was bitten by a patient which required medical treatment beyond first aid. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY PER 29 CFR 1903.19.
