29 CFR 1926.200(g)(2): The employer did not ensure that all traffic control signs or devices used for protection of construction workers conformed to Part VI of the Manual on Uniform Traffic Control Devices (MUTCD) Millennium Edition, December 2009: On or about September 6, 2023, employees were exposed to struck-by hazards on Interstate 480 Westbound in North Olmsted Township, where the employer did not have the following traffic control signs or devices in use to protect the crew that had one out of three lanes of traffic closed for edge line removal purposes on an active temporary construction zone: a) The employer did not ensure advanced warning signs used for traffic control were properly placed for a single lane closure on a multilane highway, per the MUTCD. b) The employer did not ensure traffic control devices used for traffic control were properly placed for a single lane closure on a multilane highway, per the MUTCD c) The employer did not ensure an arrow board used for traffic control was properly placed for a right lane closure on a multilane highway, per the MUTCD. d) The employer did not ensure an adequate buffer zone was installed for a single lane closure on a multilane highway, per the MUTCD, exposing employees to struck-by hazards.
