29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300: a. Viking Paper Corporation, at a worksite located at 5148 Stickney Ave., Toledo, Ohio 43612: On or about September 4, 2023, the employer failed to ensure that the OSHA Form 300 for calendar year 2023 contained a complete description in column F. The 2023 300 log did not define the object that directly caused the injury to the employee.
