OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazard of the use of damaged hooks on underhung hoists creating a possibility of employees being struck by logs: a) Hoist 1- On or about September 21, 2023, employees lifting logs weighing approximately 500 pounds utilizing Hoist #1 were exposed to struck-by hazards from a hoist hook that was not replaced when damaged. b) Hoist 2- On or about September 21, 2023, employees lifting logs weighing approximately 500 pounds utilizing Hoist #2 were exposed to struck-by hazards from a hoist hook that was not replaced when damaged. c) Hoist 8- On or about September 21, 2023, employees lifting logs weighing approximately 500 pounds utilizing Hoist #8 were exposed to struck-by hazards from a hoist hook that was not replaced when damaged. d) Hoist 15- On or about September 21, 2023, employees lifting logs weighing approximately 500 pounds utilizing Hoist #15 were exposed to struck-by hazards from a hoist hook that was not replaced when damaged.
