29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about September 20, 2023, the employer did not ensure that the company's energy control procedures were utilized to control hazardous mechanical movement of the Niagara 1600 ton press, serial number P52904, when die setters and press operators conducted activities such as, but not limited to die and coil setting exposing employees to struck by and crushing hazards. b) On or about September 20, 2023, the employer did not ensure that the company's energy control procedures were utilized to control hazardous mechanical movement of the Niagara 1600 ton press, serial number P52904, when tool room employees conducted activities such as, but not limited to sharpening die details and die punches or adjusting shims on the die exposing employees to struck by and crushing hazards. Toledo Tool & Die Company, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1303653, citation number 1, item number 1B and was affirmed as a final order on November 21, 2019, with respect to a workplace located at 105 West Alexis Road, Toledo, Ohio 43612.
