29 CFR 1910.23(b)(9): Ladders were not inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. a) Extension Ladder against Phosphoric Acid Holding Tank in Production Area; On or about August 22, 2023, and at times prior, the employer exposed employees to fall hazards, in that employees are required to use an extension ladder approximately 13 foot 1 inch in height to check the levels of the holding tanks without inspecting the ladders before use.
