29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: a) HK Cooperative DBA J.H. Routh Packaging Co. on or about June 29, 2023, at the address of 4413 Bogart Rd, Sandusky, OH: The employer did not ensure that the 110 VAC junction box was provided with a cover to prevent employee exposure to contact with electrical current from exposed open wiring. HK Cooperative DBA J.H. Routh Packaging Co was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.305(b)(3)(i), which was contained in OSHA inspection number 1635799, citation number 1, item number 1a and was affirmed as a final order on 03-01-2023, with respect to the workplace located at 4413 Bogart Rd, Sandusky, OH.
