1910.28(b)(13)(i) The employer did not ensure that each employee was protected from falling by a guardrail system, safety net system, travel restraint system or personal fall arrest system when work was being performed less than 6 feet (1.6m) from the roofs edge: On or about June 15, 2023, at the location of SA Recycling 3101 Varley Ave. SW, Canton, Ohio 44706: An employee of SA Recycling LLC., was exposed to an unprotected fall hazard of approximately 24 feet while cleaning debris from the Shredder Building rooftop and the employer did not ensure adequate fall protection systems/devices were in place. SA Recycling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.28 (b)(1)(i), which was contained in OSHA inspection number 1544499, citation number 1, item number 1 and was affirmed as a final order on 10/25/2021, with respect to a workplace located at 2020 Vanderbilt Rd., Birmingham, AL 35234.
