29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer failed to report the in-patient hospitalization, amputation, or loss of an eye to OSHA. A) Richards Plumbing and Heating Co. Inc. - 2800 Victory Blvd. Staten Island NY 10314 - An employee fell 16'9" down a manhole and was hospitalized on May 18, 2023. The employer failed to report within the 24-hour time period allotted. Employer reported the incident on May 22, 2023. Incident occurred on May 18, 2023.
