29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation, or the loss of an eye. (a) Hercules International, Inc., Huntington, WV: On May 18, 2023, an employee amputated his right thumb while operating an Erie 1600-ton forging press causing injuries that required in-patient hospitalization. The amputation and/or the subsequent in-patient hospitalization was not reported to OSHA by the employer.
