29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about April 14, 2023, an employee sustained a laceration and fracture to the left leg when struck-by a forklift resulting in an inpatient hospitalization. The employer did not report the incident to the local OSHA office until April 18, 2023.
