MCP23W00060 on the 2023 OSHA 300 log did not provide a description of the injury. b) 122 North Street, York, PA 17403 - on or about April 13, 2023, column F for case no. MCP23W00060 on the 2023 OSHA 300 log did not accurately describe the part of the body affected. c) 122 North Street, York, PA 17403 - on or about April 13, 2023, column F for case no. MCP23W00060 on the 2022 OSHA 300 log did not accurately describe the object that directly injured the person. d) 122 North Street, York, PA 17403 - on or about April 13, 2023, column F for case no. MCP23W00091 on the 2023 OSHA 300 log did not provide an accurate description of the injury. Abatement certification required within 10 days after abatement date.
