29 CFR 1910.147(c)(7)(iv): The employer's certification of employee training did not contain each employee's name and date of training: (a) On or about April 12, 2023, the employer did not certify each employee assigned tasks that required application of the control of hazardous energy was trained. This exposed employees to hazardous energy during servicing and maintenance activities.
