29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident (a) Follansbee Cleaning Fleet, 54 Landry Lane, Follansbee, WV: On March 22, 2023, during a barge cleanout, an employee was fatally wounded after being struck by the clamshell attachment of an excavator. The work-related incident was not reported to OSHA by the company within 8 hours.
