29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel. Engineering and Warehouse Areas: On and/or before March 09, 2023, the employer did not ensure removal of the power industrial truck(s) that were not in safe operating condition(s). Employee(s) were exposed to struck-by hazards. Following Equipment/Non Functional Devices: a) Komatsu - Model: FG25T-12, Serial: 554600A (nonoperational horn and right hand brake light) b) Komatsu - Model: FG25T-12, Serial: 554601A (nonoperational horn) Casino Pauma was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 1347334, citation number 1, item number 1 and was affirmed as a final order on November 20, 2018, with respect to a workplace located at 777 Pauma Reservation Rd, Pauma Valley, CA 92061.
