29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about March 7, 2023, employees installed utility pipes on the site's northeast side in an excavation where the employer did not instruct each employee in the recognition, avoidance, control, or elimination of unsafe conditions and the regulations applicable to their environment: a. Employees, installing a drainage pipe in an excavation, were not trained to recognize the hazards of working in a trench. South La Contractors LLC (DBA RedStag) was previously cited for a violation of this occupational safety, and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 1609661, citation number 1, item number 3-b, and was affirmed as a final order on March 16, 2023, with respect to a workplace located at 40070 Parker Rd, PRAIRIEVILLE, LA 70769.
