29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation, or the loss of an eye. On or about 28 February 2023, an employee of Yadi's 13 Inc was hospitalized after being injured from falling from the roof of a residential building. The employer failed to report the in-patient hospitalization to OSHA within 24 hours of the incident. OSHA learned of the hospitalization through a referral on 7 March 2023.
