29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Worksite: On or about 2/25/2023, the employer had not reported to OSHA, within 24 hours, that an employee sustained a partial finger amputation. The incident occurred on or about 2/25/2023 and OSHA was not notified until 2/28/2023.
