29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence: a) On or about February 24, 2023, and at times prior, employees in training to operate a powered industrial truck did so without the direct supervision of a person who had the knowledge, training and experience to train operators and evaluate their competence exposing employees to potential struck by hazards.
