29 CFR 1910.147(d)(4)(i): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions. Lockout or tagout device application. Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. At 209 Logistics Center Drive, Wentzville, MO 63385, On or before February 21, 2023 an employee was exposed to the hazards associated with the unexpected energization of equipment due to not affixing lockout or tagout devices to isolate energy during maintenance work to repair a Koops Booking Press Station 10. Employees did not affix an energy isolating device(s) before conducting maintenance activities. Piston Automotive was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(1)-(6), which was contained in OSHA inspection number 1310126, citation number 1, item number 1, and was affirmed as a final order on May 30, 2018, with respect to a workplace located at Belvidere, Illinois.
