29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent. (a) Operator Indoor Training Facility: On or about February 15, 2023, and times prior, the employer did not classify an injury resulting in days away from work correctly on the Log of Work-Related Injuries and Illnesses/OSHA Form 300. On February 3, 2023, an employee was working on a roof and fell 30ft. to the ground below resulting in severe injuries causing days away from work, column (H) -Days away from work and column (K)-Away from workdays were left blank on the OSHA Form 300 log.
