On or about November 10, 2022, the employer did not record the following workplace injuries or illnesses on the OSHA 300 Log for the Calendar Year 2022: Case Number 2 November 10, 2022, Machine Operator; The employee activated the machine and cut and fractured their right 3rd finger. They were away from work for 7 days and she received a job transfer/restriction for 92 days. Abatement Certification and Documentation is required
