29 CFR 1910.28(b)(3)(iv):(iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used: At a workplace located at 300 Lime Kiln Road, Green Bay, WI in the Harvest Department; Employees were exposed to fall hazards of approximately 11 feet to the floor while using a ladderway hole as a point of access to the Beef Wash Cabinet, which was not equipped with an offset or a self-closing gate. JBS Green Bay, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.28(b)(3)(iv), which was contained in OSHA inspection number 1304856, citation number 1, item number 5 and was affirmed as a final order on October 3, 2019, with respect to a workplace located at 1330 Lime Kiln Road, Green Bay, Wisconsin.
