29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: At a facility located at 502 E. Linden Street in Abbotsford, WI; the employer did not provide machine guarding to protect operators and other employees in the machine areas from hazards, such but not limited to: a) On or about December 6, 2022, in the Forming 2 area on the IR Line, the employer did not protect a Machine Operator employee from the ingoing nip points and rotating parts presented by the rotating auger of Meat Saver equipment on a MP800 forming machine. An employee suffered a fingertip amputation from contact with the unguarded auger. b) On or about December 14, 2023, in the Forming 2 Area on the IR line, the employer did not protect employees from the ingoing nip points presented by the IR oven's metal conveyor belts. c) On or about January 25, 2023, in the Forming 2 area on the Spiral line, the employer did not protect employees from the ingoing nip points presented by the metal conveyor belts of the Spiral oven where it passed over pulleys/shafts at the connection points to transfer conveyors. d) On or about January 25, 2023, in the Forming 2 area on the Double D1 (DD1) line and in the Forming 3 area on the Double D2 (DD2) line, the sausage link forming equipment (linker) was not guarded to prevent employees from coming into contact with the pneumatically driven knives of the linker equipment. e) On or about January 25, 2023, in the Forming 3 area on the Double D2 (DD2) line, and Forming 2 area on the Double D1 (DD1) line, the employer did not protect employees from the ingoing nip points presented by the metal conveyor belts at the pulleys/shafts near the catch plate/grease trap, and at the point where the oven conveyor received product from the sausage link forming equipment. f) On or about January 25, 2023, in the Fresh Kitchen area on the Brat line, Hormel Link line and Hand Pack Line, the employer did not protect employees from the ingoing nip points presented by the conveyor belts where they passed over pulleys: - located where trays of brats transferred from the blue plastic conveyor to the Omori STN Series stretchwrapper machine's white conveyor. - located where brats from the stuffers of Brat Line 1 and Brat Line transitioned to V-shaped conveyors. - located where the trays of links were manually transferred to stretchwrapper machine's white conveyor. - located where employees stacked and transferred sheets of links to the boxing process. g) On or about January 25, 2023, in the RTE-1 area, the employer did not protect employees from the ingoing nip points presented by the conveyors passing over pulleys located where the conveyor received product from the sausage peeler and at the end of the conveyor line in the boxing room. h) On or about January 25, 2023, in the RTE-3 area, the employer did not protect employees from the: - ingoing nip points on Lines 7, 6, 5, where the blue conveyors ran over pulleys to transfer boxes from ADCO packaging machines to their respective scales/metal detector equipment. - caught in the moving parts of the tray conveyor located below the Drake sausage patty sorting machines. - caught-in the rotating parts of the sausage link sorting machines on Lines 6 and 5. i) On or about January 25, 2023, in the BG-2 area at the Chub mixer, the employer did not protect employees with guarding to prevent employees from reaching into the rotating parts and caught-in hazards presented by an auger of the mixer. j) On or about January 26, 2023, in the Boning Area at the 1st Chuck line and the Loin line the employer did not protect employees from the ingoing nip points presented by the undersides of the overhead conveyors as they passed over pulleys adjacent to the bandsaw station. k) On or about January 26, 2023, in the MT-2 maintenance shop, the employer did not protect employees from the rotating part or caught-between hazards presented by the Plate Sharpener whose interlock and guard had been disabled and removed respectively. l) On or about December 14, 2022, in work areas including, but not limited to, BG-1, BG-2, BG-3, BG-4, South Kitchen, Fresh Kitchen, and Forming areas; the employer did not protect employees from caught-between or struck by hazards presented by combo lifters (dumpers) used in those departments. Employees could bodily enter or reach into the areas below the raised dumpers and then be caught-between or struck-by the dumper equipment and combo (tote) as they were lowered. The Abbyland Foods Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(1), which was contained in OSHA inspection number 1471711, citation number 1, item number 1 and was affirmed as a final order on March 9, 2021, with respect to a workplace located at 502 E. Linden St., Abbotsford, WI 54405.
