29 CFR 1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. At jobsite located at 2601 Central Ave. Dodge City, KS, the employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevation. An employee engaged in fire sprinkler removal activities was exposed to fall hazards of approximately 8.5 feet from bottom of the wooden frame support of the drop ceiling to the concrete floor below in that fall protection was not utilized.
