29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At the workplace located at 7601 East 12th Street Kansas City, Missouri64126, the employer did not report an amputation as a result of a work-related incident to OSHA within twenty four (24) hours. An employee amputation occurred on or about November 25, 2022. The amputation was not reported to OSHA until November 29, 2022.
