29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): A) On or about November 30, 2022, an employee was exposed to an approximately 18-foot soil wall while standing at an unprotected end of a trench box. B) On or about December 1, 2022, an employee was exposed to cave-ins from two unprotected walls of an approximately 18-foot-deep excavation. C) On or about December 5, 2022, an employee was exposed to cave-ins while working in an approximately 16-foot-deep excavation without protection. D) On or about January 10, 2023, an employee was exposed to cave-in hazards while working in an approximately 10-foot-deep excavation without protection. E) On or about January 20, 2023, two employees were exposed to cave-in hazards along the south wall of a 12-foot-deep excavation without protection.
