29 CFR 1910.119(e)(5):The employer did not establish a system to promptly address the PHA team's findings and recommendations; Sumitomo Bakelite North America, doing business as Durez Corporation, located at 13717 US Highway 68 in Kenton Ohio: Instance A On or about December 12, 2022 the employer did not assure that the findings and recommendations made during the process hazards analysis regarding overpressure hazards posed to occupied process buildings such as, but not limited to, building #40 control room were resolved. Employees working in the building #40 control room were exposed to significant overpressure explosion and deflagration hazards from the process area in that the control room was not equipped with appropriate blast resistant construction or other protective measures.
