29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300) and for the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: On or about December 8, 2022, and at times thereto, the employer did not maintain OSHA injury and illness forms for years 2018 to 2023. An employee suffered an injury in approximately 2021 which required medical attention; the employee missed days away work; and, the injury was not recorded on an OSHA Form 300 and/or OSHA Form 300-A, or equivalent forms.
