29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about Friday, September 30, 2022, at 12935 Cortez Blvd., Brooksville, FL 34613, the employer failed to report to OSHA within 24-hours a work-related employee amputation. The employer notified OSHA on Monday, October 3, 2022 of the amputation. Texas Roadhouse Inc., was previously cited for a violation of this occupational safety and health standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1563100 , citation number 1, item number 1 and was affirmed as a final order on May 19,2022, with respect to a workplace located at 854 I-10 Service Road Slidell, LA 70458.
