29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) a) Throughout the Facility: A Hyster Model S50XM, Serial # 0187V30031A powered industrial truck had an inoperable back up alarm and broken horn and backup lights and was not taken out of service. Observed on or about 11/16/22. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. UNITED HOSPITAL SUPPLY CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 1910.178(p)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1066120 CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON 11/18/2015.
