29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On November 10, 2022, and times prior thereto, at reactor tank number 2, the employer did not ensure fire extinguishers were identified with a fire extinguisher sign and ready accessible for employees exposing to fire and smoke hazards. a) Fire extinguisher sign missing on the west side lower deck of the reactor tank, b) Fire extinguisher sign missing above the west upper deck at the reactor tank.
