29 CFR 1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: Pettisville Grain Co. located at 1825 County Road DE in Pettisville, Ohio 43553: On or about November 10, 2022 the employer did not ensure employees were provided with basic advisory information contained within provisions of 29 CFR 1910.134 Appendix D. Employees entering grain storage bins were exposed to hazards associated with donning filtering facepiece respirators voluntarily during cleaning procedures.
