On or about November 8, 2022, at the site location of 14539 Five Point Rd. Perrysburg, OH: The employer did not develop or maintain a written record of fall protection training certification for all employees who might be exposed to fall hazards. The employer failed to evaluate and certify fall protection training was conducted by a competent person.
