29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the OSHA Form 300 or equivalent: Amazon.com Services LLC - PHL7 at 560 Merrimac Avenue in Middletown, DE 19709: On or about October 27, 2022, the employer had not provided the required description of the injury or illness, parts of body affected, and object/substance that directly injured or made person ill on the OSHA Form 300 or its equivalent for the calendar year 2022. Violative instances included but were not limited to: a) Case No. 108 (Mods) occurring on 10/14/2022 did not contain a complete entry in Column F, description for the recordable injury. Column F only contained 'AA was performing daily pick tasks'. Failing to describe the illness or injury, struck by a box that fell from upper level bin; parts of the body affected, hand/wrist; and object/substance that directly injured or made person ill, box of trash bags. b) Case No. 109 (Mezz small singles pack area) occurring on 10/12/2022 did not contain a complete entry in Column F, description for the recordable injury. Column F only contained 'AA was performing daily tasks in pack, single smalls'. Failing to describe the illness or injury, hand pain; parts of the body affected, right hand; and object/substance that directly injured or made person ill, using strong C grip while performing daily tasks.
