29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and kept up to date: a) For the yard pugmill, equipment number 701, on or about 10/14/2022 and at times prior to, the employer exposed employees to potential broken bones, amputation and laceration hazards, in that, a certification record of training on lockout tagout was not available. Pugmill equipment such as, but not limited to, belts, clutches, radiators, hydraulic lines and rotating shafts were worked on, containing hazardous energy in the form of high pressure hydraulics and rotating/moving parts from a diesel powered engine.
