29 CFR 1904.29(b)(3): The employer did not fill out or correctly fill out an OSHA Form 300 or equivalent for each recordable injury or illness within 7 calendar days of notification of an injury or illness: Powertex Sportswear, Inc. - 5651 Highway 93, Eau Claire, WI. On or about October 19, 2022, the employer had not correctly and completely entered workplace injuries or illnesses on the OSHA Form 300 for calendar year 2022. (a) On or about July 22, 2022, an employee experienced pain which resulted in job transfer or restriction. The employer did not enter the case on the form.
