29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production Floor, Sheeter line 1; On or about September 28, 2022, and at times prior to, the employer allowed employees to conduct maintenance activities on the conveyor motor without ensuring that energy control procedures were developed, documented and/or utilized, exposing the employees to electrical hazards. b) Production Floor, cut size sheeter Type SLK90, Machine # ooH80390; On or about September 28, 2022, and at times prior to, the employer allowed operators and maintenance personnel to conduct maintenance such as jam clearing at the machine # 00H80390 without ensuring that energy control procedures were developed, documented and/or utilized, exposing the employees to amputation hazards. c) Production Floor, machine model #FL200-RS-NG-AB-000, Serial # 0007-6024-2083; On or about September 28, 2022, and at times prior to, the employer allowed operators and maintenance personnel to conduct maintenance such as jam clearing without ensuring that energy control procedures were developed, documented and/or utilized, exposing employees to amputation hazards. d) Production Floor, automated conveyorized model Q-1000, Serial # 0a001433; On or about September 28, 2022, and at times prior to, the employer allowed operators and maintenance personnel to conduct maintenance such as jam clearing without ensuring that energy control procedures were developed, documented and/or utilized, exposing the employees to amputation hazards. e) Production Floor, ream accumulator 134L; On or about September 28, 2022, and at times prior to, the employer allowed operators and maintenance personnel to conduct maintenance such as jam clearing without ensuring that energy control procedures were developed, documented and/or utilized, exposing the employees to amputation hazards. f) Production Floor, Case Packer Model 86R; On or about September 28, 2022, and at times prior to, the employer allowed operators and maintenance personnel to conduct maintenance such as jam clearing without ensuring that energy control procedures were developed, documented and/or utilized, exposing the employees to amputation hazards. g) Production Floor, Pemco Hydraulic Power Unit 19-9174; On or about September 28, 2022 and at times prior to, the employer allowed operators and maintenance personnel to conduct maintenance such as jam clearing without ensuring that energy control procedures developed, documented and/or utilized, exposing the employees to amputation hazards.
