Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about September 15, 2022, at a workplace located at 1000 Morris Street, Fond du Lac, Wisconsin in the Moulding Department: employees engaged in servicing and maintenance tasks, including setup tasks, were not required to perform lockout procedures to ensure the machine's rotating components were at a zero-energy state and employees were not exposed to amputation hazards. b) On or about September 21, 2022, at a workplace located at 1000 Morris Street, Fond du Lac, Wisconsin in the Moulding Department: an employee engaged in servicing and maintenance tasks, including setup tasks, was not required to perform lockout procedures to ensure the machine's rotating components were at a zero-energy state and employees were not exposed to amputation hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
