29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about 9/15/2022 OSHA was made aware of a partial amputation accident . The accident happened on 7/29/22 and was not reported to OSHA by the employer until 9/15/2022.
