Sandusky Street Bellefontaine, Ohio, 43311: On or about September 8, 2022, the employer did not ensure OSHA Form 301 or equivalent for each recordable injury or illness was correctly filled out as required per OSHA regulations for 2019-2022 year to date, including for a hospitalization injury occurring on September 8, 2022.
