29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about September 6, 2022, with respect to an establishment located at 1400 Plover Road, Plover, WI 54467, located at Palletizing Line #15, employees were exposed to machine hazards associated with moving parts during the unjamming of a pallet from a pallet dispenser. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform the servicing work per [1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. On September 6, 2022, located at Palletizing Line #15, an employee was seriously injured while attempting to unjam a pallet without the control of hazardous energies. b) On or about September 6, 2022, with respect to an establishment located at 1400 Plover Road, Plover, WI 54467, located at Palletizing Line #11, employees were exposed to machine hazards associated with moving parts during the unjamming of a pallet from a pallet dispenser. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform the servicing work per [1910.147(d)(2) requirements].
