29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. a) On or about September 9, 2022, at the jobsite employees were engaged in work activities related to utility line installation while working in a trench approximately 5.5 feet deep. Employees were not provided a safe means of egress, a ladder, located on the east side inside the trench. Employees were climbing in and out of the trench. The Holtger Bros., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1411959, citation number 1, item number 1a and was affirmed as a final order on 01/15/2020, with respect to a workplace located at Durand Ave. and Ohio St. Racine, WI 53403.
