29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in a hospitalization, amputation or the loss of an eye: On or about June 22, 2022, at the P. R. Machinery tube bending machine, an employee suffered a finger amputation while operating the machine. The employer did not notify OSHA of the work-related amputation.
