29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. (a) Throughout the Establishment: On or about August 3, 2022, an OSHA Form 301 equivalent form for the following work-related injuries or illnesses were incorrectly filled out for the calendar years 2019, 2020, and 2021 for the following cases: (1) 2019 (4 injuries updated to 3 injures) - Incorrectly filled out an missing -Employee date of birth, time began work. There are two versions of the Associate Injury Investigation Report, one does not contain completed by phone number. (2) 2020 (3 injuries) - Incorrectly filled out and missing - One incorrectly filled out missing employee date of birth. Two injuries did not have an OSHA 301 or equivalent. (3) 2021 (2 injuries) - Incorrectly filled out and missing - One Employee date of birth, time began work, source of injury. One inury did not have an OSHA 301 or equivalent.
