29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi: a) Building #2 Tool Issue: On or about 07/21/2022, oxygen and propylene hoses were not pressure tested to at least 300psi, in that fittings were replaced and tested between 120 -200psi. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
