29 CFR 1926.1204(e)(1)(i): Employer did not perform pre-entry testing to the extent feasible before entry was authorized: On or about June 2, 2022, and at times prior thereto and thereafter, employees entered a manhole to perform construction tasks when the internal atmosphere was not tested as required, exposing employees to oxygen deficiency and atmospheric contamination hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the use of direct monitoring equipment to evaluate the atmosphere prior to allowing employees to enter spaces.
