29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: (a) The employer did not report a work-related in-patient hospitalization to OSHA within 24 hours. On June 3, 2022, an employee who was working from a six-foot A-frame ladder to install a new LED light fixture was found unresponsive on the floor by another employee. The employee was hospitalized with serious injuries. The employer reported the incident to OSHA on 6/6/22. (b) The employer did not report a work-related fatality to OSHA within 8 hours. The employee died of his injuries on June 10, 2022.
